  • The patient/ next of kin registered with AHD or legal representatives are the eligible individuals to raise a complaint. If you are neither you are welcome to use the general/suggestion form.

  • If the patient is a minor (less than 18 years old) the patient’s parent/ legal guardian/ custodian or next of kin are eligible to submit a complaint on behalf of the patient.

  • To preserve the patient’s rights and privacy this document is considered confidential for the internal use of AHD.

  • Upon receipt of the complaint, the submitter will receive an acknowledgement SMS from AHD.

  • AHD will communicate with the patient regarding the subject matter within 30 days.
